A permit to work is a formal written system for controlling high-risk work, such as work on isolated plant, hot work or confined spaces. The form is only part of it: the system needs trained issuers, clear authorisation and monitoring.
Adapt this template to your site's permit procedure. Specialised permits (confined space entry, hot work, electrical) usually need extra sections.
