How to use them
- Print on thick card, cut out and laminate, then punch the hole. Or use the layout as the artwork for a batch of durable tags.
- Every person working on the equipment fits their own lock and tag. Only the person named on the tag removes it.
- An OUT OF SERVICE tag warns people not to use faulty equipment. It is not an isolation and does not replace a lock.
In the US, OSHA 29 CFR 1910.147(c)(5) requires lockout and tagout devices to be durable, standardised and substantial enough to withstand the environment they are used in. Paper tags are suitable for training or as artwork only; for real isolations use purpose-made tags. In the UK, follow your safe isolation procedure (HSE HSG253).
Templates are a starting point. Adapt them to your site, your equipment and your procedures, and have them reviewed by a competent person.
